Job Summary

Total Health Care (THC) is committed to providing a full range of services that meet the needs of the patients we serve. This position is responsible for the collection of quality data for patients enrolled in the MDPCP and other programs to track patients and ensure compliance with data reporting. The MDPCP and Quality Improvement Data Analyst is critical to our service delivery and, is responsible for advising, monitoring and providing overall support to the management team.

The MDPCP and Quality Improvement Data Analyst position is responsible for Continuous Quality Management (CQM) which includes the development and dissemination of monthly performance reports This individual also monitors the MDPCP and Quality CQI /Performance Measurement Plan.

Essential Job Functions

  • Work with internal and external members of the MDPCP and Quality Team to support data collection efforts.
  • Follow up with staff to problem solve data collection gaps or integrity issues.
  • Assist staff with developing and following through on Plan-Do-Study-Act (PDSA) projects.
  • Ensure all reports are submitted in a timely manner.
  • Develop and maintain dashboards using tools like SQL, Excel, and scripting language to enable leaders and staff the ability to monitor data collection.
  • Develop and maintain dashboards to help track complex communication with staff across various stages of data collection process.
  • Support data verification for all data reported.
  • Document data transfer and quality assurance processes as they are developed.
  • Make recommendations concerning best practices for data collection and entry.
  • Create and maintain any necessary databases including design, documentation, extraction and deletion.
  • Assist staff with creating queries required for quality measures.
  • Test to ensure data integrity and efficient processes for data collection, entry, and retrieval.
  • Maintain security and confidentiality of patient/ protocol information.
  • Works in collaboration with management to optimize data management and functionality/ reporting of quality measures

Job Title: MDPCP andQuality Improvement Data Analyst

Assumes primary responsibility for concurrent data collection and entry on specified data points

  • Track and ensure data is submitted timely and validate for accuracy.
  • Review requested data to determine scope and availability of information.
  • Participates with the management team on data driven decision making and planning.
  • Identify data requirements and perform structured/random quality audits across multiple software applications to validate accuracy of data/data sets, interfaces, and work queues.
  • Assists with design and creation of new data report and collection tools.
  • Independently creates new reports in existing database tools.
  • Track various data errors and complete analysis to recommend correction.
  • Create materials and trainings to train staff in high quality data collection and submission methods.
  • Promote positive internal and external customer relations through prompt and courteous service.
  • Participates on organization and departmental committees for Performance and Outcomes.
  • Other duties as assigned.

Minimum Education And Experience Required

  • Bachelor’s Degree, concentration in Informatics/Information Systems preferred
  • Management of information systems required.
  • Basic clinical knowledge preferred.
  • 1-3 years of work experience in a professional, data-driven environment
  • Experience in using Excel and SQL to manipulate data
  • Experience with developing PDSA’s.
  • Experience in one or more of the following: quantitative data collection, cleaning, processing, and data management accountability practices.
  • Demonstrated project management experience and excellent communication skills.
  • Background in both data management and analytical skills in healthcare environment.

Required Knowledge, Skills And Abilities

Possess knowledge in quality performance and their applicability to outcomes management.

  • Excellent customer service skills and the ability to communicate effectively; must be able to work as part of a collaborative team.
  • Ability to prioritize tasks and deal with frequent interruptions/ requests effectively.

Remains current on external trends related to quality measures.

  • Competently utilizes computer hardware and software programs including but not limited to: Microsoft Windows, Microsoft Word, Excel, Access, SQL
  • Being open to change and considerable variety in work activities.
  • Analytical ability sufficient to evaluate data, make judgments and recommendations.
  • Ability to independently work through details of a problem to reach a positive solution.
  • Excellent verbal, written and interpersonal communication skills.
  • Ability to differentiate between primary and secondary source systems and databases.
