The Role

We are seeking an experienced Epic Healthy Planet Analyst for a fully remote, direct-hire opportunity with one of our top clients. This is a hospital system we have partnered with for years, giving us valuable perspective on their culture, leadership expectations, and the type of experienced professionals they are seeking.

If you have deep expertise in population health, risk stratification, quality measures, and value-based care initiatives, this is a role where you can make a real impact.

What You'll Do

• Design, build, test, and support Epic Healthy Planet functionality including risk scoring, registries, and quality measure reporting.
• Configure and maintain population health tools such as Patient Registries, Member Dashboards, and Risk Stratification models.
• Collaborate with clinical, operational, and analytics teams to improve population health outcomes.
• Troubleshoot and resolve complex Healthy Planet issues.
• Participate in system upgrades, optimizations, and new module implementations.
• Document build changes and maintain clear configuration records.

What We're Looking For

• Active Epic Healthy Planet certification.
• 3+ years of hands-on Epic Healthy Planet build and support experience.
• Strong understanding of population health workflows, risk adjustment, and quality measures.
• Experience with payer and value-based care initiatives.
• Ability to work independently in a fully remote environment.
• Strong communication and stakeholder collaboration skills.

Nice to Have

• Additional Epic certifications in Ambulatory, MyChart, or Care Everywhere.
• Experience with Epic's Value-Based Care modules.
• Prior consulting or project leadership experience.
