The Montfort Academic Family Health Team is recruiting a Manager, Quality, Data and Collaborative Networks (18-month contract, remote within Ontario), seconded to AFHTO for the duration of the contract. In this role, you'll work at the intersection of quality improvement, data, organizational excellence, and system collaboration, helping primary care organizations translate evidence and insights into meaningful improvements.
Working closely with AFHTO members and partners across the province, the successful candidate will support provincial initiatives, learning networks, quality improvement activities, and member engagement efforts that strengthen integrated, team-based care.
