Description:
- This position requires the analyst to have a Healthy Planet certification from Epic and ideally:
- · Experience with Compass Rose / advanced risk stratification tools
- · Experience integrating external data sources (claims, SDOH, HIE feeds) into Healthy Planet models
- · Strong background in quality measure logic (HEDIS, eCQMs)
- · Familiarity with care management documentation and utilization workflows
Their day-to-day responsibilities could include:
- · Configure, optimize, and support Epic Healthy Planet build, including registries, risk scoring models, care gap logic, and patient attribution frameworks
- · Perform detailed analysis of population health workflows (e.g., care management, outreach, quality reporting) and translate operational needs into system design and enhancements
- · Develop and maintain registries, reporting workbench reports, dashboards, and analytics to support value-based care initiatives and quality programs (e.g., HEDIS, Medicare Stars)
- · Partner with Population Health leadership and clinical teams to identify care gaps and implement system-driven interventions to improve quality and cost outcomes
- · Design, build, and maintain care management tools including care plans, protocols, assessment tools, and outreach workflows within Healthy Planet
- · Support payer and value-based contract requirements by ensuring accurate data capture, attribution logic, and measure calculation within Epic
- · Analyze and troubleshoot data integrity issues related to population health metrics, registry inclusion/exclusion criteria, and reporting discrepancies
- · Collaborate with data and analytics teams to validate data flows between Epic, Clarity/Caboodle, and downstream reporting platforms
- · Lead or support optimization efforts to improve user efficiency, including workflow redesign, automation opportunities, and enhanced reporting solutions
- · Participate in governance and prioritization processes for Population Health enhancements, ensuring alignment with organizational goals and regulatory requirements
- · Support testing, validation, and change management activities for Healthy Planet upgrades, new releases, and enhancement deployments
- · Provide end-user support and issue resolution for Population Health stakeholders, including care managers, quality teams, and operational leaders
- · Assist with onboarding of new programs or service lines into Healthy Planet, including registry design, measure mapping, and workflow alignment
- · Maintain documentation for build standards, workflows, and reporting logic consistent with IT and Population Health governance standards
